Community Adult Disability Services

The Community Adult Physical Disability Team provides a range of support based on assessed need to enable all those within our service to maximize independence through a person centered approach.

The Community Team comprises of:

  • Social Workers
  • Care Managers
  • Assistant Care Managers

The team provides statutory social work support and care management for services users with a physical disability between the ages of 18 to 64 years.


  • Adult Physical Disability Day Services

    Adult Physical Disability Day Care Services provide support to adults between the ages of 18 and 64 years with a range of complex physical disabilities including individuals with a sensory loss and acquired brain injury.

    The provision of day care support enables Service Users to continue to live within their local communities, enables access to a range of community services and also provides support to families with caring responsibilities.

    Day Service facilities promote a person centered approach with an individual tailored programme based on each persons need and best outcomes.
