Community Rehabilitation services provide intermediate rehabilitation to service users over 18 years of age in their own homes/care homes. The service assists in preventing hospital admissions, timely discharge from hospital, maximises independent living, and supporting people to live longer in their own homes.
The team consists of AHP Physiotherapists, Occupational Therapists and Rehabilitation Assistants.
Interventions for the majority of service users will last up to six weeks (though there will be individual exceptions). The service does not provide rehabilitation to specialised areas such as stroke, or mental health rehabilitation.

Referrals
The service accepts referrals from GPs, hospital, health and social care professionals and other trusts. Hospital discharges with a social worker involved are made via the social workers in the respective trusts.
A direct referral to the service can be made via:
- ENCOMPASS (NI Trust Staff with access)
- Trust Staff (no ENCOMPASS access) – email CommunityRehab@setrust.hscni.netwith professional reports (Physiotherapy and Occupational therapy) when discharge confirmed.
- GPs – e-mail Community.Rehab@setrust.hscni.netwith clear history indicating potential benefit from rehabilitation.
