Hospital Social Work

Hospital Social Workers help patients, carers and families to deal with the consequences of illness and disability by providing services, information and support.

The Hospital Social Worker will assist patients to organise services required for discharge to ensure a safe and timely discharge.

Patients unable to return home will be assisted to find alternative accommodation, such as nursing or residential home. This may include an initial placement to enable the patient and their family or carer to plan future care needs.

The Hospital Social Worker can also access Intermediate Care Services for those patients requiring rehabilitation or convalescence upon discharge.
