Following assessment at the Mental Health Assessment Centre, if the Community Mental Health Team is identified as meeting your needs, then allocation to the Community Mental Health team will happen with 6-9 weeks (approximately).
The team offers time-limited and sometimes longer term interventions depending on your assessment of need.
Community Mental Health Teams are made up of a range of staff, including Support Workers, Social Workers, Community Psychiatric Nurses, Doctors and Occupational Therapists.
Your key worker will arrange an appointment to see you.
During your engagement with the Community Mental Health Team you will develop a Personal Well-being Plan.
This will be delivered in partnership with you and you can retain a copy and this can be shared with others if you wish.
The Personal Well-being Plan will outline how your strengths and personal abilities will be developed in helping you to resolve and or manage difficulties. The plan will set out specific actions which will help realise your expectations and personal goals. It will identify those treatments and interventions that meet your emotional and physical health needs. This will include therapies, medications and health and well-being actions.
Your Well-being plan will promote personal safety by addressing risks which may undermine your physical, psychological and social well-being. It will include those things you wish to happen if you are experiencing a crisis or setback.
Your Personal Well-being Plan will include actions that will strengthen family, social, occupational life including any supports being provided by family, friends /advocates.
This will be regularly reviewed and updated with you.
