We provide a service Trust wide to those individuals who are 65 and older who permanently reside within a Care Home setting. We provide support to the individual and their families by assessing individual need, developing a person centered support plan for each service user in partnership with the Care Home. The team work in partnership with the Care Home management and staff to ensure that the individual’s needs are met by providing clinical advice and training, reviewing care provided and providing tailored support as required.
The core team is made up of key workers including social workers, nurses and Clinical Nurse Facilitators who as part of their role provide a Tissue Viability service into the Nursing homes. In addition we have aligned Allied Health Professionals for specialist support such as a Dietitian, Physiotherapist, Occupational Therapist and Speech and Language Therapist.
The Permanent Placement Team will monitor, support and review the safety, quality and experience of SET service users who are permanent residents in a Care Home.
At present we also offer support to the Care home sector through the Trust Covid-19 Care Home Response Hub which provides support and guidance to homes when staffing is under significant pressure due to the COVID-19 crisis. The Hub provides Nursing and Health Care support staff, Patient Experience cleaning services, Infection Prevention Control and Manual Handling support & training and co-ordination of a range of other specialist services and support with outbreak testing.
