Staff from the South Eastern Trust have been honoured with a prestigious Integrated Care Award for their ground-breaking work in modernising healthcare practices. Assistant Director of Quality Improvement, Dr Ruth Gray and the Domiciliary Care Team were presented with the Award at the 24th International Conference on Integrated Care (ICIC24) in recognition of their pioneering approach to redesign, incorporating ecosystem mapping, digitalisation and co-production methodologies.
Dr Gray and her team have demonstrated exceptional leadership and vision in spearheading a transformative journey aimed at redefining the delivery of Domiciliary Care services. The award-winning initiative demonstrates the team’s dedication to continuous improvement and innovation within the South Eastern Trust. By using ecosystem mapping techniques, they have gained invaluable insights into the complex factors influencing patient outcomes, allowing for more targeted and effective interventions.
The Domiciliary Care team have embraced digitilisation that has revolutionised service delivery, streamlining processes and enhancing accessibility for both patients and service users. Through the implementation of these digital technologies, they have helped empower individuals to take a more active role in managing their own health, leading to improved outcomes and greater satisfaction.
Central to their success has been the principle of co-production, which emphasises the importance of collaboration and partnership between healthcare providers, patients and the local community. By actively involving stakeholders in the design and delivery of services, Dr Gray and her team have provided a sense of ownership and accountability, resulting in more responsive and patient focused solutions.
