
Patients leaving hospital who require oxygen therapy are being supported to return home safely thanks to the dedicated work of the Acute Home Oxygen Team at the Ulster Hospital.
The service, which began in June 2021, is delivered by a skilled team of Respiratory Nurse Specialists and Respiratory Physiotherapists. Operating Monday to Friday across the South Eastern Trust’s three hospital sites, the team plays a vital role in assessing patients for home oxygen, prescribing therapy safely and ensuring a smooth transition from hospital to home.
Since its launch, the Acute Home Oxygen Team has received over 1,000 referrals and provided new oxygen prescriptions to more than 650 patients.
South Eastern Trust, Advanced Nurse Practitioner, Rosie Thom explains, “The Home Oxygen Team provides an essential service for patients who need oxygen therapy when being discharged. Oxygen is a medicine, it must be prescribed, monitored and reviewed carefully, as having too much or too little can seriously affect a patient’s condition.
“Many patients are successfully weaned off oxygen before discharge. But for others, ongoing oxygen support is necessary to help them manage their condition and maintain their quality of life at home.”
Rosie added, “We work closely with the Community Home Oxygen Teams to continue care after discharge, whether it is in outpatient clinics or at the patient’s home.
“One of the most rewarding parts of the job is reassuring patients that oxygen is not the end of the road, it is actually a new beginning. With the right support, they can continue living life to the fullest.”
